External Description:

The Clinical Pharmacist SWHP provides all managed care clinical pharmacy-related services to help ensure effective and efficient operations within the SWHP Prescription Services department.  The Clinical Pharmacist SWHP responsibilities include drug evaluation and support of formulary additions, compliance with regulatory and quality requirements, drug utilization management activities, medication therapy management and the provision of chronic disease management services. The Clinical Pharmacist SWHP participates in implementation of innovative solutions, new services, programs, practices or lines of business to ensure appropriate pharmaceutical utilization, improve patient outcomes, and control costs.
The ideal candidate for this position must be able to work in a team environment and have strong interpersonal and problem solving skills with commitment to professionalism and providing quality care to members.
